• If you are an Authorized Representative, you will be required to submit your name, address, email or phone number as well as the full name, address, email or phone number associated with the account of the DermSilk customer you are making a request on behalf of. DermSilk will use the customer’s information to search our systems and determine if we have information about the customer. If we are able to locate information about the customer, we will fulfill the Personal Information and/or Deletion request. As an Authorized Representative, the Personal Information report will be sent to the physical address you have given us with a signature required upon delivery.
• DermSilk will satisfy deletion requests through de-identification, which will be completed within 45 days from the date of the initial request. If additional time is needed, we will inform you.
